Yoga Release Form

1. Meredith Hart agrees to furnish yoga therapy sessions, yoga lessons, meditations, breathing practices and instruction and the use of its facilities designated for that purpose at any said and agreed upon physical location, which includes online.

2. In consideration for such private sessions, lessons and instruction, the applicant agrees to pay a fee as established by Meredith Hart. This agreement, or the payment of any fee, in no way obligates the student/client to make any future payment or to take any further instruction extending beyond the period of his or her payment. Such student/client realizes and agrees that all fees when paid are non-refundable for any reason and the agreement may be revoked by Meredith Hart at any time.

3. The applicant warrants, represents and acknowledges that Meredith Hart has fully informed him or her of the nature and risks involved in the practice of yoga conducted and taught by Meredith Hart and that he or she is physically and mentally fit to participate in such activities.

4. The applicant agrees that he/she has informed Meredith Hart of all known physical conditions including pregnancy, organ transplant, medical conditions, and medications, and the applicant further agrees to update Meredith Hart with any changes.

5. The applicant understands that there shall be no liability on the practitioner’s part due to the applicant’s forgetting to relay any pertinent information. The applicant further agrees that in further consideration for the yoga instruction and therapeutic yoga sessions furnished by Meredith Hart that he or she assumes all risk of any and all accidents, injuries, or death of any kind sustained by her or him by any reason of or in connection with said activities whether such an injury or accident be sustained at any location, including online, and the applicant hereby releases, discharges and absolves Meredith Hart of any and all liability or responsibility for any accidents, injuries, or death, whether the same are caused by or attributed to their negligence or the negligence of any of them.

6. The undersigned student/client also agrees to assume the risk of any accident, injuries or death of any kind sustained any place on the premises of the above said location including, but not limited to the treatment room, studio, or any place in locations including the buildings of which said is a part, or on entering or leaving such buildings and hereby releases, discharges and absolves Meredith Hart and its agents and employees of and any liability for any accidents, injuries, or death.

7. Meredith’s approach to yoga is an original method of hatha yoga that weaves in elements from trademarked and authorized lineages. To ensure the integrity and quality of this unique method, and to protect the interests of this specialized approach, unauthorized use of the teachings in any way is prohibited. Unauthorized use - by one who is not certified - this includes but is not limited to any public performance, demonstration, or teaching - even to family and friends.

8. Because this system is highly specialized and individualized, the below signed client/student agrees to not disclose the methods, practices and protocols they have been given during class, workshops, private sessions, and online programs to others. The undersigned will abide by the rules and regulations set forth by Meredith Hart. The applicant consents to the use of and waives any compensation whatsoever for, all pictures, videos, photos, recordings, movies, tapes, TV, media coverage etc by Meredith Hart. The undersigned student and/or client agrees that all covenants and agreements to and between Meredith Hart.
